Jeffrey D. Perkins, MD PhD Clinical Director

Jeffrey D. Perkins, MD PhD, Chief Executive Officer

Dr. Perkins is a native of Houston, Texas and received his MD and PhD degrees from University of Texas Health Science Center at Houston. He received post-graduate residency training in internal medicine from Johns Hopkins Hospital in Baltimore, Maryland and completed his rheumatology fellowship training at University of Colorado Health Sciences Center in Denver. Dr. Perkins is a member of the Alpha Omega Alpha medical honor society, is board certified in both internal medicine and rheumatology and is an Assistant Clinical Professor at University of Colorado Health Sciences Center. Dr. Perkins is a Certified Clinical Densitometrist. Dr. Perkins sees patients in our Longmont and Broomfield offices. He founded Colorado Center for Arthritis and Osteoporosis in 1998.


Joseph R. Lutt, MD Director of Bone Health

Joseph R. Lutt, MD, Clinical Director

Dr. Lutt is a native of Nebraska and received his medical degree from the University of Nebraska Medical Center. He completed his internal medicine residency and his rheumatology fellowship at Oregon Health and Science University in Portland. Dr. Lutt is board certified in both internal medicine and rheumatology and sees patients in our Boulder and Longmont offices. He joined CCAO in 2009 and has been our Clinical Director since 2015.

He has particular interest in clinical research and leads CCAO’s clinical research program. His clinical interests include the management of osteoporosis, inflammatory arthritis, and autoimmune diseases. He enjoys working closely with other specialists in managing complicated diseases of the immune system affecting the eyes and skin.


Nguyet-Anh (Theresa) Tran, MD

Nguyet-Anh (Theresa) Tran, MD

Dr. Tran is a Nebraska native and received her MD degree at The University of Tennessee. She completed her internal medicine residency at Wake Forest University and her rheumatology fellowship at University of Washington in Seattle. Dr. Tran is board certified in rheumatology. In her years as a fellow at University of Washington, she developed a reputation as an outstanding clinician, and her efforts as Chief Rheumatology Fellow resulted in significant enhancements of the UW clinical training program. Her clinical interests include the diagnosis and treatment of inflammatory arthritis and, in particular, caring for the geriatric population. Dr. Tran joined CCAO in 2009 and sees patients in our Broomfield and Longmont offices.


Shiraz Moinuddin, MD

Shiraz Moinuddin, MD

Dr. Moinuddin is a native of Tennessee and received his MD degree at The University of Tennessee. He completed his internal medicine residency and rheumatology fellowship at George Washington University in Washington, D.C. Dr. Moinuddin is board certified in rheumatology. Dr. Moinuddin joined CCAO in 2010 and sees patients in our Broomfield and Boulder offices.

In addition to being an outstanding clinical rheumatologist, Dr. Moinuddin adds expertise in musculoskeletal ultrasound and sports medicine to CCAO. During his medical training Dr. Moinuddin also participated in programs to help the underserved populations in several Latin American countries, using his fluency in Spanish as well as his medical skills to good effect.


Jill E. Gibson, MD

Jill E. Gibson, MD

Dr. Gibson is a native of Aurora, Colorado and spent her formative years in Missouri. She received her MD degree from Washington University in St Louis. She continued on at Washington University for her internal medicine residency and rheumatology fellowship. Dr. Gibson is board certified in internal medicine and rheumatology. During fellowship her research interests included infectious hepatitis and its effect on inflammatory arthritis. Clinical interests include the diagnosis and treatment of autoimmune disease, particularly inflammatory arthritis and systemic lupus erythematosus. Dr. Gibson sees patients in our Wheat Ridge and Longmont offices. She joined CCAO in 2010.


Prateek Chaudhary, DO

Dr. Chaudhary received his medical and public health degrees from University of North Texas Health Science Center at Fort Worth. He completed his internal medicine residency at The University of Texas Medical School in Houston and his rheumatology fellowship at Duke University Medical Center in Durham, NC. He is board certified in internal medicine and rheumatology. Dr. Chaudhary sees patients in our Wheat Ridge and Broomfield offices. He joined CCAO in 2013.


Melissa A. Wells, MD

Melissa A. Wells, MD

Dr. Wells is a native of Overland Park, KS and received her MD degree from the University of Kansas School of Medicine. She completed her internal medicine residency at the Mayo Clinic in Rochester, MN and her rheumatology fellowship at Duke University in Durham, NC. Dr. Wells is board certified in internal medicine and rheumatology. During fellowship, her research interests involved evaluating quality measures in rheumatology. Dr. Wells sees patients in our Wheat Ridge and Broomfield offices. She joined CCAO in 2016.


Nicole S. Swavely, MD

Nicole S. Swavely, MD

Dr. Swavely is a native of Charleston, South Carolina. She received her MD from Medical University of South Carolina. She then completed her internal medicine residency at the University of Illinois at Chicago and her rheumatology fellowship at Northwestern University. During her fellowship, she studied the use of ultrasound in the diagnosis and management of rheumatic conditions. In addition to ultrasound, her other major clinical interest is geriatric rheumatology. Dr. Swavely sees patients in our Wheat Ridge and Broomfield offices. She joined CCAO in 2017.


Jonathan R. Kruger, MD

Jonathan R. Kruger, MD

Dr. Kruger is a Kansas native. After completing his undergraduate training at Union College in Nebraska, he attended medical school at Loma Linda University School of Medicine, graduating in 2010. Dr. Kruger received his postgraduate training in both internal medicine and rheumatology from University of Utah in Salt Lake City. He will join CCAO in 2017, having spent the last two years in private practice in Fort Collins. Dr. Kruger will see patients in our Longmont office.
