Rheumatology First Visit

Step 1: Patient Health Questionnaire

For your first visit, please complete the patient health questionnaire, which is the first step in your treatment. Please take time to fill out this form carefully and completely, as this information will provide a starting point for collecting information necessary for diagnosis and treatment. Step 2: First-Visit Checklist

For your first visit to Colorado Center for Arthritis and Osteoporosis, please make sure you…

  • Bring your completed patient health questionnaire. You must have it in order to be seen by a provider.
  • Bring your insurance card(s).
  • Bring your medical records (if you have been asked to bring them).
  • Are prepared to pay your insurance co-pay.
  • Arrive 15 minutes before your scheduled appointment time to assure that new patient registration and co-pay are completed in time for your appointment with the provider.

Step 3 : What to Expect During Your First Visit

At your first visit we will collect your patient health questionnaire (see Step One above). We will also ask you to provide insurance information and other demographic information that is necessary for us to bill your insurance. Please bring your insurance card(s) and referral information (if applicable) with you.

During your time with the provider, he or she will spend time discussing your symptoms and health history. Following your conversation, the provider will perform a complete physical examination. This generally does not include a breast or genital exam unless specifically indicated by your symptoms.

The provider will then talk about his or her findings with you, and explain recommendations and treatment options. Additional blood tests, x-rays, or other studies may also be ordered as appropriate. If necessary, a follow-up appointment will also be scheduled.
